미국의사면허 취득의 제한 사항들

미국(United States of America)는 말 그래도 아메리카 대륙의 state라고 불리는 독립적인 50개의 나라들이 모여 만든 연방국가입니다. 국방, 외교, 경제 일부(통화 등)를 빼고 우리가 실생활에서 정부기관과 마주하는 대부분의 일(교육, 세금, 보험, 의료 등)들은 주(state)의 영역에서 이루어집니다. 전문가 집단 관리도 다르지 않아서, 의사나 변호사에 대한 자격증명서도 연방정부가 아닌 주정부에서 발급하고 자격 유지를 위한 관리도 주정부에서 합니다. 그래서 USMLE를 치르고 의사자격증(ECFMG Certificate이 아닌 State License를 뜻합니다)을 받기 위한 조건도 주마다 다릅니다. 오늘은 USMLE를 통과하고 의사 주면허를 발급받기 위한 조건이 주마다 어떻게 다른지를 살펴보겠습니다. 

1. 면허 발급을 위한 조건들 

면허 발급을 위한 조건들은 크게 세 가지로 나눌 수 있는데요, 크게 전공의 수련 년수, USMLE 응시 횟수, 그리고 USMLE 시험 완료 기간으로 나눌 수 있습니다.

가. 전공의 수련 연수(minimum postgraduate training)

미국에서는 전공의 수련을 postgraduate training이라고 하는데요, 정확히는 레지던트 뿐만 아니라 펠로우(전임의) 수련도 모두 postgraduate training이라고 합니다. 주면허를 발급받기 위해서는 대부분 주에서는 외국 의대 졸업자에게는 3년의 수련 기간을 요구합니다. 물론 여기에서 요구하는 postgraduate training은 한국이 아닌 미국의 병원에서 받은 수련을 뜻합니다.

나. USMLE 응시 횟수

우리나라에는 의사 국시 시험 응시 횟수에 제한이 없지만 미국의 많은 주에서는 USMLE 각 Step 응시 횟수에 제한을 둡니다. 많은 주에서 각 Step을 3번 이내로 응시하여 통과하도록 요구하고 있습니다.

다. USMLE 시작에서 Step 3 마칠 때까지 걸린 연수(years)

많은 주에서 USMLE Step 1, 2CK, 2CS, Step 3를 모두 통과하기까지 7년의 시간 제한을  둡니다. 당신이 Step 2CK, Step 1, OET, Step 3 순으로 시험을 보았다면 Step 2CK 점수가 발표된 날로부터 Step 3 점수가 발표된 날까지가 7년 이내여야 합니다.

외국 의대 졸업자들에게는 이것이 조금 복잡해집니다. 바로 ECFMG에서 규정하고 있는 7 year rule 때문인데요, ECFMG에서는 첫 USMLE을 통과한 후 ECFMG 자격증을 받기까지(즉 Step 1, 2CK, 2CS를 모두 통과하기까지) 7년 이내여야 한다고 규정하고 있습니다. 만약 첫 시험을 Step 2CK로 2016년 1월 1일에 통과하였고 Step 1을 2019년 7월 1일에 통과하였으나 Step 2CS를 2024년 3월 1일에 통과하였다면(즉 첫 시험이었던 Step 2CK를 통과한 후 8년 2개월이 지났다면) Step 2CK 점수는 무효화 되어 Step 2CK를 다시 치뤄야 합니다. 그러면 유효한 첫시험은 2019년 7월 1일로 리셋되어 Step 2CK를 2026년 7월 1일까지 통과해야 ECFMG 자격증을 취득할 수 있습니다. 이 ECFMG 7-year rule를 준수하기 위해 외국 의대 졸업자는 이를 감안하여 시험 계획을 짜야 합니다. Step 3를 준비하고 치를 여유를 갖기 위해서는 적어도 Step 1, 2CK, 2CS를 6년 안애 통과하는 것이 바람직할 것입니다.

다음은 미국 모든 주 및 역외 영토에서 요구하는 제한 사항들을 정리한 표입니다.(원본에 있는 Osteopathic 면허에 대한 사항은 한국 의사들에게는 해당하지 않으므로 여기에서는 생략하였습니다.)

An unlimited license may be granted to currently enrolled residents in a post graduate training program in the U.S. or Canada, that have completed at least 24 months of progressive post graduate training and meet all requirements for an unlimited license in the state of Nevada, including having passed all three (3) steps of USMLE within the time period allowed by NAC 630.080 and commit in writing to the Nevada State Board of Medical Examiners that they will complete the program and provide satisfactory completion of the program within sixty (60) days after the scheduled completion of the program.
